Replacement Certificates

Holders of RIPH qualifications issued within the last 10 years who have lost their original certificates may apply to the RIPH for replacements. RIPH will check its records to confirm the original award of the qualification to the applicant, the date of the award and the grade, before preparing a replacement certificate.

Procedure

Please complete a “Replacement Certificate Application Form” (downloadable via the link below) and submit to the Qualifications Department together with the required documentation and a cheque for the appropriate amount made payable to 'RIPH'.

1. Current driving licence (copy)
   
2. Current passport (copy)
   
3. Birth certificate (copy)
   
4. Current official registration of the applicant with a recognised school or college for a named course of study (copy)
   
5. An official letter from the applicant's employer confirming the applicant's identity
   
6. Two utility bills issued to the applicant within the last six months

Applicants must send two of the above.

In addition, applicants who have changed their name since taking the qualification must provide proof of their name change, for example, a copy of the marriage certificate, or a copy of deed poll document. The replacement certificate will bear the name given at the time that the qualification was gained.
